What do you need to have handy when your file Form 471?

I promised a quick list of items you should have nearby when you're ready to start Form 471

  • Copy of your 470 - check and make sure that the allowable contract date has passed before you sit down to create your 471.
  • Previous year's 471 - you can use it for FSCS code and FCC registration number. You can also get these from the agency websites via January 2012 blog post.
  • Your PIN that will allow you to certify electronically. If you've lost yours, contact Client Services Bureau. If you're new to the program and haven't yet filed an E-rate form, you'll need to do at least one paper certification before you're issued a PIN.
  • School lunch data - available online from OPI. Link from January 2012 blog post. There may be more recent data available from Suzanne or your school. Make copies of whatever school lunch and enrollment data you use and note where you got it and the school year it covers.
  • Winning service provider bid or a copy of a previous bill from current service provider. This gives you the numbers to complete Block 5.
  • Some information on the services you're requesting, especially when requesting broadband services. DSL or cable? Fiber optic? Advertised speed? These are requested in the new Item 24 section. They're collecting data on the impact of the program.
